This document is to give permission for me to share your birth experience with DONA to go towards my certification.
This is to evaluate my services provided to go towards my DONA certification. This requires two people to fill out. One will be the client (aka the birthing person) and the other will be a witness (spouse/partner or anyone who was present for the birth).
